In _krb5_extract_ticket() the KDC-REP service name must be obtained from
encrypted version stored in 'enc_part' instead of the unencrypted version
stored in 'ticket'.  Use of the unecrypted version provides an
opportunity for successful server impersonation and other attacks.

Implement __cxa_thread_atexit and __cxa_thread_atexit_impl. This
functions are used for destructors of thread_local objects.

If a pending destructor exists, prevent unloading of shared objects.
Introduce __dl_cxa_refcount interface for this purpose. When the last
reference is gone and the object has been dlclose'd before, the
unloading is finalized.

Ideally, __cxa_thread_atexit_impl wouldn't exist, but libstdc++ insists
on providing __cxa_thread_atexit as direct wrapper without further
patching.

Catch up vs_set_params() to recent MI audio (after in-kernel mixer).
Before that, MD part had to support all encodings I'd like to support,
but currently it's no longer necessary.  The hardware is
4bit/1ch/15.6kHz ADPCM but it behaves as 16bit/1ch/16.0kHz PCM.

For audio.c < 1.362, the device attach succeeded and playback is
still working.
For audio.c >= 1.363, the device attach fails again.
It does not work yet but I commit it for milestone.

Remove codes for PACKET_TAG_IPSEC_IN_CRYPTO_DONE

It seems that PACKET_TAG_IPSEC_IN_CRYPTO_DONE is for network adapters
that have IPsec accelerators; a driver sets the mtag to a packet
when its device has already encrypted the packet.

Unfortunately no driver implements such offload features for long
years and seems unlikely to implement them soon. (Note that neither
FreeBSD nor Linux doesn't have such drivers.) Let's remove related
(unused) codes and simplify the IPsec code.
