Keep track of what appear to be live Fibre Channel disks and store up the
Port WWNs for them as well as attaching them as a "port-wwn" property to
the device node in question. This allows MD code to find the boot device
on some platforms.

This is less clean than it should be. A better solution would be to make
some changes to the scsi infrastructure so that periphs can query and
use and store their own "native" transport addresses. However, that's
a much more invasive change and it is not clear how many ports or devices
really want or need that information (yet).

ATA sepcs changed the LBA48 boundary from 0xfffffff to 0xffffffe between
ATA6r1 and ATA6r3, which caused drives conform to ATA6r3 or later to
reject LBA28 commands at sector 0xfffffff.
Get the right idea from the LBA48 boundary from IDENTIFY words 60-61.
Remove the WD_QUIRK_FORCE_LBA48 quirk, associated tables entries and
autodetect code, it's not needed any more.
Based on patch sent to teck-kern by Christoph Badura, use of words 60-61
instead of a constant for the LBA48 boundary by me.

for the signal setup code (etc).

- on FC-AL disks, use wwn instead of target index to match a particular drive
- use the new device_register_post_config() to match "sd" device against

Don't always use ".set mips3" - that explicitly uses 64-bit instructions
and we may be on a 32-bit CPU.  Instead use .set mips3/mips32/mips64
depending on current build arch.

Should fix boot problems on a Alchemy CPU reported by KIYOHARA Takashi
on port-mips.

A couple of niggles/concerns:

The Dell DRAC5 gives us a zero-length report immediately following
a normal report.  Thus, ignore zero-length reports.  Move some related
report size insanity checking into the UHIDEV_DEBUG case.
Should fix PR/39911.

Process flush requests from the file server in a separate thread
context.  This fixes a long-standing but seldomly seen deadlock,
where the kernel was holding pages busy (due to e.g. readahead
request) while waiting for the server to respond, and the server
made a callback into the kernel asking to invalidate those pages.
... or, well, theoretically fixes, since I didn't have any reliable
way of repeating the deadlock and I think I saw it only twice.

The VATTR_NULL/VREF/VHOLD/HOLDRELE() macros lost their will to live
years ago when the kernel was modified to not alter ABI based on
DIAGNOSTIC, and now just call the respective function interfaces
(in lowercase).  Plenty of mix'n match upper/lowercase has creeped
into the tree since then.  Nuke the macros and convert all callsites
to lowercase.
